262 CORE Outreach Logger

This web application lets community outreach workers track their efforts and their clients. It provides access to structured forms for collecting information about community members. It also provides forms for recording the types and amounts of assistance and services that interviewees received. As the workers use the app, it also logs their location and the time of day for the purpose of mapping outreach efforts and tracking efficacy.

Privacy Policy

This app requires a login with an email address.
It maintains a whitelist of authorized users that stores user’s email address and preferred name.
It then accesses that user’s email address and runs as a script with their identity.
When used to track outreach activity, it will collect physical location and time of day data that it will store and use for reports.
Specific data about user location and time will only be shared for internal 262 CORE process control. It will not be shared with any other third party.

Higher Expectations will not share any personally identifiable information about either:
1) users of this app

2) clients who are described in the data that this app records

with anyone other than the 262 CORE team and Community Power Consultants

Terms of Service

Users of this software must complete training through the Medical College of Wisconsin so that they are collecting data about community members in an informed and responsible way.

Higher Expectations is not responsible for any misuse of community information by its users.

Higher Expectations does commit to storing the data in a secure cloud file with access strictly limited to their data team and authorized individuals from 262 CORE and Community Power Consultants.

The data collected by this software will be use to create aggregate reports to policy makers and granting agencies. These reports may include information about the time and locations where users collected information. The data will also be used for internal continuous improvement by 262 CORE and Community Power Consultants.
